First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options.  Occasionally, surgery can be performed the same day as the consultation.  However, a complex medical history or treatment plan or the need for general anesthesia, for example, will require a first consultation appointment and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

-Your referral slip from your referring dentist and any x-rays if applicable. 
-A list of medications you are presently taking and medical problems you may have.
-If you have medical or dental insurance, please have all necessary information before calling for your initial appointment.  This will save time and help you in expediting the insurance reimbursement process.  Please bring in your medical or dental insurance information for your first visit so that we can make copies for your chart.
 
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian.

A preoperative consultation and physical examination is mandatory for patients wanting or needing intravenous anesthesia for surgery.

 
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).  Having a copy of your most recent History & Physical Examination from your physician/physicians to provide to us is very important for patients who have complicated medical problems.
 
X-rays
If your dentist or physician has taken x-ray(s), you may request that the x-ray(s) be forwarded to our office.  If there is not enough time, please pick up and bring the x-ray(s)  to our office.  Copies of x-ray(s) are usually of too low in quality to be useful for Dr. Woo, unless they are electronic copies that are forwarded to us in electronic format.  If additional radiography are necessary, they can be taken at our facility.
